Hi.  I had a total hysterectomy 20 years ago.  I have never had a clear smear test since due to HPV being persistent. 18 months ago I went for my routine gynae apppointment, every 6 months, to be find out (after smear and biopsy) that I have VIN1 and mild dyskaryosis (again something I have had for 20 years).  In December 2020 I had another colposcopy and smear.  Today I have found out that my mild dyskaryosis is now ‘moderate’ and they want to see me in the colposcopy clinic in three weeks, instead of the normal 6 months, where they will check the vaginal wall and may do biopsies (am sure they will).  My mind is going to fear of progression and that I will eventually get cancer.  Any one out there with any ideas…..

I am sorry to hear about the ongoing issues you have had with persistent HPV over the years. I can see where your concerns about cancer are coming from but remember screening and follow up appointments are all about the prevention of cancer by picking up abnormality at an early stage that can then be monitored or treated.

At your next colposcopy appointment as you know they will be able to take another look at both the cervix and the vulva and if needed some biopsies will be taken. This will guide the team as to what needs to happen next. Whilst mild dyskaryosis is usually just monitored any moderate or severe cell changes will often than not be treated by removing the abnormal areas. You can read more about this here.

VIN 1 is often found in people who have had persistent HPV for many years but through regular monitoring hopefully any further changes can be picked up and treated to reduce the risk of abnormal cells turning into cancer similarly to cervical precancerous cells. Whilst I appreciate your anxiety the important thing is you will continue to be closely monitored to ensure any changes are noted and treated to try and reduce the risk of you getting these types of cancer.

Thank you so much Naomi.  Although my objective head tells me this is all preventative sometimes it feels as if I am going backwards.  However, I know that because I am checked so regularly and treatments are available anyway, I am extremely lucky.  This just reminds me of the reason for getting checked.  It just serves to remind me how glad I am that I have been looked at since my hysterectomy.  

You have been very kind to answer me as my anxiety overrode my logical brain yesterday.  Feel better today.
